PLEASE NOTE:  Effective October 1, 2009, marriage and family licensure board statutory changes signed by NC Governor Beverly Perdue on July 31, 2009  were implemented.  These changes incorporated the addition of a provisional licensure option (LMFTA)  and options for those seeking reciprocal MFT (licensed in other states) licensure. Effective September 15, 2009, new forms were posted at this website.

These are the forms you should use to apply for licensure in NC.  If you have previously downloaded other forms they should be discarded.  Effective September 15, 2009 only applications utilizing the new forms will be accepted.
